Patient Care

  • Provide home visit to conduct an initial assessment for needs.
  • Help you make medical decisions, review options, conduct research as necessary into your diagnoses, and translate into language that you and your family can understand.​
  • Assist with appointment scheduling and develop calendar for appointments.
  • Accompany you or your "family" (as defined by you) to clinic appointments, rehabilitation or other outpatient services.
  • ​Accompany the patient to the hospital for surgery or procedures.
  • Educate the patient and family as needed to understand medical terminology, diagnoses and federal requirements for care.
  • Find additional specialists for second opinions.
  • Assist with discharge planning and determine best placement opportunities along the continuum of care inclusive of home services.
  • Assist with end of life planning.

Quality of Care

  • Assure quality of care:  does it meet evidence-based standards and federal, state and regulatory standards?
  • Determine if there are any discrepancies in care that could meet the definition of a "medical error."
  • Help with ethical dilemmas that may be presented to an organizational ethics committee.
  • Review medical records and compile a personal medical record.
  • Prepare medical records for potential legal or regulatory reporting or follow-up.
